Homemaker Personal Care Professional
Horizons Inc. provides HPCP, Homemaker Personal Care Professional. This person will provide day to day care to individuals and teach them to function in their environment, whether at home or in the community. This self help goals are clearly outlined in the individuals ISP ( Individual Service Plan) and include, personal care, financial management, medical, safety, community involvement and residential.

Residential Services
Residential services allows Horizons Inc. Staff to provide individualized services that fit the special needs of each person. Under this living alternative support services can be provided for the individual in the home of their choice with hours scheduled as needed.
Horizons provides services to the individual in any of the following areas:
- Health
- Safety
- Personal income management
- Housing
- Habilitation
- Transportation to community activities, socialization, etc.

Adult Day Services
Enjoy participating in different activities
Horizons Inc. provides an array of daily services to our Adult Day Care Center, helping our community members live their lives with dignity, purpose, and meaning.
What is Avenues of Adventures?
Avenues of Adventure is an adult day service program. This program offers life-enhancing services in a warm and interactive environment for individuals with developmental disabilities living in Tuscarawas County. Those coming to the center will meet people and develop life skills through classes and community based activities.
Participants will have fun while fostering a sense of self-worth and taking part in new experiences designed to encourage independence and growth.

Direct Support Professional
DSP, Direct Support Professional, this person will provide direct care and supervision to the individuals in an intermediate care facility (ICF) . The kind of self- help training for the individual is outlined in their ISP (Individual Service Plan). This includes accompanying the individual on medical appointments and community activities.

Training & Development
Horizons Inc. is committed to training and developing employees to ensure quality service is provided to the individuals we serve. All employees who provide services to the individuals are REQUIRED to complete training for certification in the following areas:
- Direct Support Professional
- CPR/AED/First Aid
- Medication Administration
- MUI/UI Training- (Major Unusual Incident/Unusual Incident)
- Non violent Crisis Intervention
- Fire and Emergency Response Training
We strive to make the developmentally disabled individuals as independent as possible. It is our mission to create home-like residential settings for these individuals while providing them with a high quality of life.

Transportation Services
Horizons Inc. is also part of a collaborative transportation effort with the Tuscarawas County Senior Center, Society for Equal Access, Access Tusc to provide transportation to our individuals.
