Horizons of Tuscarawas and Carroll Counties, Inc. began offering services in 1976. Our agency began in the heart of an aging parent whose concern for her adult developmentally disabled child was to keep her from being placed in an institution after the parent passed away. The parent wanted the child to reside in her own home. Horizons Inc. is a private, non profit organization providing services to meet the needs of people with developmental disabilities within Tuscarawas and Carroll Counties. We began with 24 individuals and today we serve over 70 individuals with developmental disabilities within residential settings.
HORIZONS INC AN AGENCY PROVIDING SERVICES TO INDIVIDUALS WITH DD
Horizons Inc. is a private,non profit organization providing services to meet the needs of people with developmental disabilities. Horizons Inc. currently provides services in six counties:
* Ashland
* Coshocton
* Harrison
*Jefferson

SERVICES PROVIDED
Horizon Inc. is committed in the field of providing services for DD Individuals,
we employ over 178 people to provide the needed services of the individual.
Horizons Inc. can accommodate any hours of services needed by the individual. Whether it be 24 hours a day, 7 days a week or just a few hours a day, week or month, we provide the necessary training for our staff to assist the individual.
Horizons Inc. provides HPCP, Homemaker Personal Care Professional , this person will provide day to day care to individuals and teach them to function in their environment, whether at home or in the community. This self help goals are clearly outlined in the individuals ISP ( Individual Service Plan) and include, personal care, financial management, medical, safety, community involvement and residential .
DSP, Direct Support Professional, this person will provide direct care and supervision to the individuals in an intermediate care facility (ICF) . The kind of self- help training for the individual is outlined in their ISP (Individual Service Plan). This includes accompanying the individual on medical appointments and community activities.
TRAINING AND DEVELOPMENT
Horizons Inc. is committed to training and developing employees to ensure quality service is provided to the individuals we serve. All employees who provide services to the individuals are REQUIRED to complete training for certification in the following areas:
*Direct Support Professional
*CPR/AED/First Aid
*Medication Administration
*MUI/UI Training- (Major Unusual Incident/Unusual Incident)
*Non violent Crisis Intervention
*Fire and Emergency Response Training
We strive to make the developmentally disabled individuals as independent as possible. It is our mission to create home-like residential settings for these individuals while providing them with a high quality of life.
TRANSPORTATION SERVICES
Horizons Inc. is also part of a collaborative transportation effort with the Tuscarawas County Senior Center and Society for Equal Access to provide
transportation to individuals with developmental disabilities, low income populations, senior citizens, and others throughout Tuscarawas County in need of transportation services. This collaborative effort is called the Tusc.Transit Transportation Program and is partially funded by a grant through the Ohio Department of Transportation.

Horizons INC. is hiring for FULL-TIME, PART-TIME HPCP's at all of our Group Homes! ALL SHIFTS!
********$250.00 SIGN ON BONUS after 90 days, Another $250.00 after 180 days!********
FLOAT POSITION
A float must be able to do the following:
- must work anytime needed up to 40 hours per week, including midnights, weekend days, early morning shifts, etc.
-must work at a variety of different sites. Some sites may have individuals with challenging behavior; some need to be lifted, some will need total care regarding personal hygiene such as changing attends, etc.
-A float must not work over 40 hours in a week unless approval is given by the Executive Director or her designee (Area Coordinator, Program Manager Supervisor, Program Coordinator, etc.).
- When you have worked over 40 hours in one week, you must report this to the person calling you in to work so they can find another replacement when necessary.
-The float should not be scheduled for long periods of time at any one site (over two weeks) without prior approval from the Executive Director or designee.
-The float will be paid the following benefits: (a) Insurance coverage for the individual minus the cost paid by each employee; (b) one week paid vacation per year; (c) five sick days paid per year; (d) mileage to and from work site.
-A float can work a total of 40 hours per week and some approved overtime, or no hours per week. There could be a period of time where the float is not needed and they would have no hours.
-The float must keep track of his/her own hours. The supervisor will be the Area Coordinator, Program Manager Supervisor, or Program Coordinator as designated by the Executive Director. This appointment will depend on the number of hours the float person works in an area.
-The float must be trained at various sites. He/she is responsible for doing all paperwork as set forth in the ISP of the individual with whom they are working.
-Employees in the Float Position who do not fulfill the obligations as listed in this job agreement will be removed from the position as a float.
-There is a difference between a float position and a sub position. A sub can refuse work and a sub can work with whom they desire when they desire. A float must work a variety of places as called in and can only refuse once he/she has worked 40 hours in one week.
-The float must carry a pager or cell phone for easy access, and must promptly respond to all calls.
-The float must be responsible for keeping time sheets for each individual or site where he/she works.
-The float must be responsible for submitting time sheets at the end of the pay period to Payroll.
-The float must attend all in-services and training sessions as requested by supervisor.
-The float must keep current CPR and First Aid, MUI/UI training, Medication Administration training, etc.
-The float must notify his/her supervisor when services are refused by an individual or by a family member.

The Homemaker/Personal Care Professional will empower adults with developmental disabilities while supporting them daily. One must have the ability to work as a team player. Be able to define problems, collect data, establish facts and draw valid conclusions; handle sensitive face-to-face contacts, maintain a friendly atmosphere and be willing to have fun! Must have a valid driver's license. Will be required to transport individuals for activities, doctor's appointments, etc. Must have adequate transportation to complete job responsibilities. Training provided.
Qualifications: High school diploma or GED preferred but not required for position. Must have evidence of the ability to work constructively with developmentally disabled individuals. Previous direct care, nursing and / or developmental disabilities experience preferred. Must demonstrate basic respect and good will toward individuals and co-workers. Must possess good general health and be able to lift or help loft adults. Must have a valid driver's license. May be required to transport individuals for activities doctor's appointments, etc., in a 12 passenger van with a wheelchair lift.

Weekend Warrior HPCP- this person will have the choice of 12 or 16 hour shifts Friday, Saturday and Sunday only.

Be able to define problems, collect data, establish facts and draw valid conclusions; handle sensitive face-to-face contacts, maintain a friendly atmosphere and be willing to have fun!
